City Community Team are seeking a Community Nurse based at Five Rivers Health and Wellbeing Centre. Teams within Wiltshire Health & Care provide high quality, complex care in the community setting, which includes caring for some very vulnerable people often in extremely challenging environments.

Main duties of the job
Experience in front line NHS community patient care is essential as is evidence of up-to-date clinical professional development. Applicants need to have excellent clinical skills, well developed communication skills and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated, and hardworking individual who can work flexibly and efficiently.
City Community Team is a busy Team caring for patients registered with a South Wiltshire GP. You will have the opportunity to work alongside other members of the Multidisciplinary Team such as Occupational Therapists, Physiotherapists, Assistant Practitioners and other Specialist Practitioners including Consultant Practitioners and Advanced Clinical Practitioners. Our Shift patterns are 7-3, 8.30-4.30 and 2-10pm.

Qualifications
* Registered Nurse
* To possess a current, valid full driving licence
* Evidence of continuing professional development
* Knowledge of legislation affecting professional practice
* Experience of applying research based information to practice
* Teaching and Assessing in Clinical practice
* Further qualifications relevant to the role
* Non-medical prescribing course
* Awareness of recent Government Papers that may affect future clinical practice
Experience

* Evidence of innovative and flexible approach to care and the organisation of care

Additional Criteria
