We are looking to appoint a motivated and enthusiastic AHP Clinical Lead (Band 7) with proven skills in the management of older adult patients with rehabilitation needs.
The successful candidate will lead the therapy input within Glan Irfon Reablement/ Rehabilitation Unit, Builth Wells, working alongside the Registered Manager.
Glan Irfon is a 12 bedded unit providing short term assessment or care planning to individuals that require wraparound 24 hour support and rehabilitation that cannot be provided at home.
The unit provides an exciting opportunity to develop the Discharge to Recover then Assess (D2RA) model, recognising that the acute hospital setting does not provide a suitable environment for recovery and assessment for ongoing needs.
Responsibilities will include:
1. Service delivery and development of Glan Irfon
2. Admitting and Discharging individuals
3. Training and Development of staff (Care Provider/PTHB)
4. Trusted Assessor for Social Care Assessment, including Reablement and Residential care home placement
5. Assessment, Intervention and delegation of Rehabilitation Plans
Main duties of the job
Work as an autonomous practitioner delivering therapy and manage patientswith highly specialised complex needs in a variety of care collaboratively with other professions, GPs, Consultants, Nurses andother AHPs to provide expert patient centred care.Provide training, education, supervision and appraisal for staff and students.Contribute to the development of professional practice and the transformationand evaluation of service delivery.Support the research and innovation agenda through participating in researchand development aspects of service evaluation with an emphasis on value-based care andcomplex audit within their in partnership with service users and staff to achieve improved healthoutcomes for service users and develop clinical pathways.
