Job summary Our Enablement and Urgent Care services are responsive and flexible with the aim of supporting people to develop and maintain their independence within their homeenvironment. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals.Supervising a team of Support Workers, as a Home First Assessor you will work closely with people who use our services, their families, friends and carers, who all play aninvaluable role in how our users experience our services. They are expected to be kind and responsive, professional and informative and contribute to the quality of the servicesprovided across the Health and Social Care economy Main duties of the job Ensuring the best possible outcomes by using up-to-date skills and adhering to evidence-based policies and procedures.o Prepare and present cases at a weekly meeting with the Registered Practitioner ensuring that every aspect of the persons journey through the service is available to ensure ajudged decision based on up to date information.o Manage a caseload of people requiring enablement/assessment, and community equipment assessment, identifying the Service Users' eligible needs and developing anenablement support programme in conjunction with individual Service Users, their families, carers and external agencies in order to enable the Service User as much as possiblewithin this short, time-limited intervention.o Manage a caseload of clients requiring long term domiciliary services and developing a support programme in conjunction with individual service users, their families, carersand external agencies.o Monitor provision and quality of service through regular visits to Home First Support Workers at their workplace, to ensure work undertaken consistently meets clientrequirements in accordance with each programme of support, enablement policy and national standards of care.Please refer to the attached Job Description for full job specification.
