Red Snapper Recruitment are recruiting for a temporary case work coordinator working with a company that provides supportive housing solutions to individuals who have a recent history of homelessness and that have been recently released from prison.
The Case Work Coordinator is expected to hold day to day responsibility for managing referrals, assessing and supporting clients in a way that allows them to maintain accommodation and make positive changes in their life. You are expected to develop effective and productive relationships with key partner agencies such as National Probation Service, Police and other criminal justice and generic support services that facilitate production of multi-agency support planning. This is a temporary position lasting 4 - 6 weeks with the possibility of extension. It is part time working 30 hours per week, paying £12.50 per hour.
Main duties and responsibilities
1. Build positive and effective working relationships and agreements with relevant external agencies which will include working closely with criminal justice services.
2. Progress and track referrals made into the service and outcomes for service users moving on from service.
3. Complete needs assessment/risk assessment for clients leading to the delivery of key elements of support plans which meet individual client need and reduce re-offending.
