THIS POST IS FOR 31 hours a week. 2 shifts 7PM-7AM and 2 shifts 7PM-11PM per week.
Come and join our CQC rated 'Outstanding' Trust as a Community Staff Nurse in the Out of Hours District Nursing team. As a Community Staff Nurse, you'll help deliver care to people in their own homes and get to work with inspiring, skilled teams whilst you do it. The Out of Hours District Nursing team works overnight from 7pm to 7am. This Post is for 31 Hours a week. This will be different nights and can be discussed at interview.
The OOH team covers District Nursing in Berkshire overnight.

Our community nurses work closely with members of the wider multi-disciplinary teams, including our GP's, local hospices, complex care nurses, health and social care coordinators, specialist nurses and therapist colleagues.

Main duties of the job
1. To work in conjunction with the Clinical/Team Leader, Community Nursing Band 6 and all staff within the team, to holistically assess the needs of patients and carers, and to work in partnership with the patient and family to plan, implement and evaluate programmes of nursing care.
2. To be able to undertake first visits and take any immediate action, reporting back to the Clinical/Team Leader, Community Nursing Band 6 and Community Matron for further discussion and/or assessment.
3. To establish working relationships with other professionals and voluntary agencies to achieve health aims.
4. To undertake practice assessor and supervisor training support students and new staff to the service.
5. Provide care according to the available evidence of best practice and the support of others in the development of evidence - based practice.
6. To be actively involved in promoting self-care for patients and building Health Promotion into care.
7. To demonstrate knowledge and skills including assessment, reassessment, medication management, bladder bowel care, tissue viability, care, use and administration of treatment using specialist equipment, palliative care and symptom control, managing acute episodes of are, reducing hospital admissions, as per band 5 competency document.

The must haves for this role:
21. Registered Nurse /Adult Nurse Trained along with complete NMC registration (or in the instance of a final year nursing student able to provide these prior to their agreed start date for the role)
22. To work bank holidays/weekends or unsocial hours to meet the needs of the service.
