Community Rehabilitation Specialist Dietitian/ Full Time 37.5 hrs per week

An excellent opportunity has arisen for a dynamic and enthusiastic Dietitian with a passion for nutrition support in the community to join our team. The successful candidate will work under the guidance of the Team Lead Dietitian and alongside other team members to provide specialist dietetic expertise for adult patients in patient’s homes, clinic setting, nursing and care homes, community hospitals, virtual wards and group sessions.
Main duties of the job
As a Band 6 Community Dietitian, you will provide expert nutritional support to individuals across the community, helping to manage chronic conditions, promote healthy eating, and improve overall wellbeing.
What We’re Looking For:
* Registered Dietitian with HCPC registration.
* Experience in community or clinical settings (Band 5 or above).
* Strong communication skills and a compassionate approach.
* Ability to work independently and as part of a multidisciplinary team.
* Passion for promoting public health and nutrition.
* Must have a drivers licence and access to a vehicle.

Key Responsibilities:
* To be responsible for effective time management, meeting deadlines and responding to fluctuating workloads.
* To have responsibility for the organisation of a clinical caseload to adults in their own homes and community care settings.
* To be accountable for own professional action and recognise professional boundaries, seeking advice as appropriate.
* To advise head of service on issues of service delivery including clinical activity, service pressures versus service capacity.
* Arranging and delivering training to other health professionals within the remit of the service level agreement.
* To develop the service linked with community rehab teams and liaise with community nurses within these groups.
* To work closely with supporting the virtual frailty ward.

* Experience of working within the NHS setting.
* Experience of working in a community dietetic environment post qualification.
