What you will do:
* Undertake assessments of citizens in line with statutory duties under relevant legislation.
* Plan and commission services that help adults overcome barriers and achieve personal outcomes. Engage in co-production with citizens and carers to review care plans.
* Work collaboratively with citizens, carers, and at-risk adults to provide opportunities for them to lead fulfilling lives in ways that matter to them.
* Coordinate applications for resources and work with internal and external partners to support citizens and carers.
* Manage a specific caseload, prioritising workload effectively.
* Maintain accurate electronic case files and other documentation for performance management purposes.
* Participate in supervision and personal development reviews, identifying necessary training to maintain your registration with Social Care Wales.
* Undertake training for continuing professional development, including potentially becoming an Approved Mental Health Practitioner or pursuing senior roles.
* Mentor other staff or students, contributing to training programmes and participating in team meetings when necessary.
* Participate in duty rotas and activities associated with a multidisciplinary team.
* Adhere to Council’s policies and procedures, including Health and Safety and Equal Opportunity guidelines.
* Report any safeguarding concerns promptly to the designated officer.
Requirements:
* Full compliance with Social Care Wales standards and ongoing registration.
* Over 3 years post-qualification experience.
* A commitment to the Council’s Environmental Policy and Equal Opportunities Policy.
* Ability to work flexibly and as part of a fast-paced multidisciplinary team environment.
