We currently have two substantive Band 3 Clinical Support Worker working in the community. Our team work together to support each other as well as giving you the autonomy to work with individuals, develop a therapeutic relationship and support individuals to live as independent and fulfilling life as possible We are a recovery focussed, multi-disciplinary team providing an effective, supportive and professional service for our Services users and Carers. We want to build on and continue to improve services so we can be the safest trust with gold standard practices. Main duties of the job You will undertake the planning, with the support of a registered professional and delivery of direct patient care for your caseload, enabling service users, requiring secondary mental health services to develop independence in the community and access the right resources to support their recovery. This will be with supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
