Job summary We are looking for an enthusiastic and motivated individual to join our team for a period of maternity leave for 12 months, working on a full time basis to take responsibility for administration / IT matters relating to Malvern Town PCN. We are seeking a Care Co-ordinator who will play a key role alongside members of Malvern Town PCN. Care Co-ordinator's work closely with Primary Care Network (PCN), GP Practices to ensure that their service needs, and targets are met. This new role aims to provide administrative support to members of Malvern PCN Team working to ensure the effective delivery of care, accurate record keeping, collating data and administrative support to all services relating to patient care. Main duties of the job Main Responsibilities are to: Provide proactive operational and administrative support. This will include collating data and information, ensuring accurate record keeping, liaison with patients, health, and social care professionals, dealing with enquiries and providing information as required. Work with Malvern PCN team members e.g. the Care Home Team and Physiotherapy colleagues to ensure accurate and up to date records are maintained on EMIS and other related database systems. Produce reports from the data systems for the GP Practices to support service development and evaluation. Liaise with all staff across the Primary Care Network to promote good communication and ensure delivery of effective and efficient services.
