The Health Practitioner Service has been an established part of Bolton's primary care workforce for over a decade. During this time, we've helped tens of thousands of patients to prevent illnesses like type 2 diabetes, cardiovascular disease, and COPD through our 12 month, evidence-based lifestyle intervention.
As our Deputy Service Manager, you will support a team of Health Improvement Team leaders and Social Prescribing Link Workers, all of whom work across 49 GP surgeries in Bolton.
This is a demanding, but rewarding role in what can be a rapidly evolving professional landscape. You will be well supported by the Service Manager and those around you, but the ability to cope with and lead others through complex change is crucial for this role.

Main duties of the job
1. Develop and maintain effective working relationships with the Band 7 Service Manager, HIP Team Leaders, SPLWs, and other HIPs.
2. Develop and maintain effective working relationships with primary care and Integrated Care Services Division colleagues.
3. Develop and maintain an extensive knowledge of the HIP role.
4. Act as the main administrator for the Data Collection and Reporting System (DCRS).
5. Work collaboratively with the Team Leaders to ensure robust reporting systems, including data input.
6. Plan, implement, monitor, and evaluate robust evaluation systems for the service as defined by the Service Manager.
7. Support other team members with research, audits, and evaluations as required.
8. Support the Public Health Programme Manager for Health Improvement and Primary Care Development with the development of new wellbeing initiatives for primary care.
9. Assess and evaluate complex data. Liaise when appropriate with FT management/GMICP management/Commissioner.

Job responsibilities
The Deputy Service Manager will provide support to the Service Manager in providing leadership and management for the delivery and provision of the NHS Health Improvement Practitioner Service across Bolton.
Health Improvement Practitioners (HIP) are a key part of the local health improvement workforce, tackling health inequalities and supporting the agenda to increase life expectancy of the Bolton population.
HIPs are based within GP surgeries across the Borough and work alongside primary care colleagues.
This post will be expected to support and provide a comprehensive reporting function, developing support systems to enhance service efficiencies and support performance management. The role will facilitate a robust evaluation function for the service, subsequently monitoring the outcomes and producing analytical reports as required.
An essential part of this remit is to work strategically across the local health & social care economy, collaborating and negotiating with public health, primary care, and social care. This collaboration will ensure the service is meeting exacting standards of service delivery and there will be successful progression of ad hoc projects and pathways.
The role will require an effective planner with exceptional organisational skills. The post holder will work autonomously across a range of projects and across organisational boundaries to support a dynamic and progressive HIP Service.
As part of the overall duties of the post, this will involve generic HIP duties and tasks, including managing their own caseload of patients and providing a comprehensive behaviour change service for targeted groups of patients within primary care.
Person Specification
Experience
* Has worked within primary care or has experience of primary care practice.
* Has experience of healthcare work with patients in health improvement.
Qualifications
* Health trainer or equivalent qualification.
* City & Guilds NVQ Level 3 Health Trainer Qualification.
* Qualified to Degree level or equivalent level of experience.
* ECDL or equivalent experience.
* Leadership qualification or working towards this.
