Clacton PCN has an opportunity for a digital care co-ordinator to join our team on a full-time basis up to 37.5 hours per week. The role is for an initial period of one year but with proven results we will be looking to make this a substantive role. The role of the PCN digital care coordinator is to support patients and the practice teams to maximise the potential presented by innovations in digital healthcare technology and the associated new ways of working to ensure we address health inequalities and provide equitable access to all our patients. This is an innovative and exciting role, which involves using available data sources to understand the health and well-being profile of our patients. It requires working within a team of PCN and practice staff to support with relevant new technology, data analysis and our digital maturity. Above all it is about making a positive difference to the lives of our patients. The main duties of the role include: Act as an ambassador for the use of digital healthcare technology with both patients and colleagues. Identify and support those who currently do not use digital channels for health access to make better use of available digital technology, promoting digital inclusion. Support the PCN and practices with handling and analysing data reports and developing our digital maturity. Support practices to maintain and monitor data coding and quality on clinical systems ensuring coding is compliant and consistent. Optimise use of PCN One Drive, website and social media platforms in conjunction with the digital transformation lead. Support the introduction of new technology including the education of patients and staff both face to face and remotely. Collate and develop promotional material to improve uptake of existing and new technology. Support the PCN team with any relevant projects and tasks and attend PCN meetings when required. Act as first point of contact for all digital enquiries. Support practices and the PCN with development of reports on both Systmone and EMIS. About the Candidate We are looking for an enthusiastic, meticulous and proactive individual who is keen to work as part of a team across our six practices. You will be self-motivated with well-developed communication skills and excellent IT literacy. Ideally you will have experience of working in health care settings. The ideal applicant will have the following skills, experience and qualifications: Experience of working within a healthcare setting. Strong listening and communication skills. Professional qualification or relevant experience in IM&T. The ability to deliver technical information in a non-technical way to a range of stakeholders. Communicate effectively in a range of situations across a range of patients, colleagues and stakeholders.
