We have an exciting opportunity at the Market Harborough Medical Centre for a Practice Nurse to join our large Clinical team. This is an exciting new opportunity to join a busy, highly regarded, supportive and forward-thinking GP training practice.

The purpose of this post is the delivery of high quality primary care nursing services to the practice population. You will work with practice policies, protocols and guidelines whilst using experience and clinical judgement to provide nurse interventions and care.
Main duties of the job
Previous experience within a primary care setting would be an advantage, although full training will be given to suitable applicants.
* Have experience within diabetes clinics
* Working within the NMC code, the post holder will deliver care that is evidence-based and cost-effective.
* To be responsive to patients' needs and contribute to improving quality standards.
* Work collaboratively with the multi-disciplinary general practice team to meet the needs of the patients.

Market Harborough Medical Centre is in the centre of a busy market town and is one of three practices including St Lukes and Husbands Bosworth. We have a large list size of patients with a team of Partners and Salaried GPs. We have an experienced Nursing Team and a variety of additional roles including Physicians Associates. We have a very active and engaged PPG.
Job responsibilities
* To work independently whilst accepting professional accountability with the scope of your clinical practice and expertise.
* Sign post effectively to other clinicians and agencies when appropriate.
* To work evening and Saturday sessions as required by the Practice.
* Promote health and well-being through the use of health promotion, health education and screening.
* Apply infection control measures in all areas of practice.
* Undertake clinics and offer focused lifestyle advice in line with local and national guidance and Quality Outcomes Framework indicators.

Person Specification
Qualifications
* Registered General Nurse (Registered with the Nursing & Midwifery Council)
Experience
* Recent and relevant experience in primary care
* Experience in the management of patients with Long term conditions
