You will work as an integral member of the podiatry and diabetes foot protection teams providing highly specialist care for the high-risk and most complex cases. Based in the community, with a domiciliary caseload, the role will also be part of the wider MDT diabetes foot team at Ashford and St Peter's Hospital. You will participate in consultant-led MDT Diabetes foot clinics and Podiatry/Diabetes Nurses led Hot Foot Clinics delivering high quality, innovative and responsive programmes of podiatric care. There will also be an element of MSK as this is critical to long-term care for the patients.
Experience of managing high-risk diabetes patients, preferably in an acute MDT, is required, as well as possessing relevant post-graduate studies/qualifications. Although not essential, it would be desirable to be a non-medical prescriber.

Main Duties of the Job
1. Provide highly specialist diagnostic assessment, treatment and advice for high risk, ulcerated and the most complex cases.
2. Be responsible for the day-to-day running of podiatry clinics in the community and in hospital sites, cascading information to the whole team, triaging referrals and requesting investigations.
3. Be a part of the wider multidisciplinary diabetes foot team (MDT and Hot foot Clinic) at Ashford and St Peter's Hospital providing podiatry care for both inpatient and outpatient services.
4. To be responsible for maintaining patient pathways between acute and community care.
5. To lead and implement a regular review of outcome measures across the service to ensure the service provided is effective.
6. To be involved in the operational and strategic development of the service, embracing developments in practice and evidence-based medicine.
7. To act as a source of expertise and undertake training of podiatry students, help colleagues maintain and develop their clinical skills in wound care, and educate other health care professionals on the role of podiatric wound care.
8. To participate in podiatry service wide CPD activities, clinical audits, service pilots/projects and other research activities to support the development of evidence-based practice.
