Main duties of the job
Clinical
a) Patientsupporting role to ensure effective medicinesuse, through shareddecision-making.
b) Medicines optimisation tasks -effective medicine administration (e.g. checkinginhaler technique), supporting medication reviews and medicines reconciliation. Utilise consultation skills to work with patients toensure they use their medicines effectively.
c) Medicationreviews and medicines reconciliation for new care home patientsand synchronising medicines for patient transfers between care settings, linking with local community pharmacists, andreferring to the pharmacist for structured medication reviews.
d)Provide specialist expertise, to both thepublic health and social care needs of patients, including lifestyle advice,service information, and help in tackling local health inequalities.
e)Supporting initiatives for antimicrobial stewardship to reduceinappropriate antibiotic prescribing.
Technical and Administrative:
a) PCN multi-disciplinary team toensure efficient medicines optimisation and implementing efficient orderingand return processes and reducing wastage.
b)Providing training and support on the legal,safe and securehandling of medicines,including the implementation of the Electronic Prescription Service (EPS).
c)Processing monthly PPA claims.

Duties and ResponsibilitiesClinical
a) Undertaking patientsupporting roles to ensure
effective medicinesuse, through shareddecision-making conversations with patients.
b)Carrying out medicines optimisation tasks includingeffective medicine administration (e.g. checkinginhaler technique), supporting medication reviews and medicines reconciliation. Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively.

e)Taking a centralrole in the clinical aspectsof shared care protocolsand liaising with specialist pharmacists for more complex patients.
f)Supporting initiatives for antimicrobial stewardship to reduceinappropriate antibiotic prescribing.
Technical and Administrative:
a)Working with the PCN multi-disciplinary team toensure efficient medicines optimisation, includingimplementing efficient orderingand return processes and reducing wastage.

c)Developing relationshipswith other pharmacy technicians, pharmacists and members of the multi-disciplinary team to supportintegration of the pharmacy team across health andsocial care including primary care, community pharmacy, secondary care andmental health.
d)Supervising practice receptionteams in sortingand streaming general prescription requests, so as toallow GPs and clinical pharmacists to review the more clinically complexrequests.
e)Processing monthly PPA claims.
The role will also require pharmacytechnicians to supportthe implementation of nationalprescribing policies and guidance withinGP practices, care homesand other primary care settings. This will be achieved through undertakingclinical audits (e.g. use of antibiotics), supporting quality improvementmeasures and contributing to the Quality and Outcomes Framework and enhancedservices. In addition, pharmacy technicians will assist in the delivery ofmedicines optimisation incentive schemes (e.g. medicines switches) and patientsafety audits.
Pharmacytechnicians will provide leadership for medicines management systems across PCNs, supporting practices with a range of services to get the best value from medicines byencouraging and implementing Electronic Prescriptions, safe repeat prescribingsystems, and timely monitoring and management of high-risk medicines).
Training requirements
All pharmacytechnicians must have completed or be enrolled in, be undertaking or be preparedto start an approved 18-monthtraining pathway (e.g.Primary care pharmacy educational pathway (PCPEP) or Medicines Optimisation inCare Homes (MOCH)). Pharmacy technicians must be registered with the GeneralPharmaceutical Council. Entry to the PCPEP programme will include the optionfor an accreditation of Prior Learning (APEL) process.

* Awareness of general practice and the agenda for medicines optimisation
* Experience of working in Primary Care

Qualifications
* Professional registration with GPhC
* BTEC/NVQ level 3 or equivalent in pharmaceutical sciences
* Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
* Evidence of continued professional development (CPD)
* Postgraduate qualifications of a relevant nature eg FdSc in Medicines Management/Pharmacy Services; BTEC Therapeutics
