We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. Main duties of the job Assessing mental health crises in individuals referred to the service and supporting the delivery of safe and effective home treatment for an identified client group and in accordance with a wellbeing and recovery action plan You will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved. You will assist in the delivery of a high standard of nursing care to service users referred to the team. You will implement agreed systems for the monitoring of effectiveness of care. You will participate in the education and management of junior staff.
