Job overview We currently have exciting opportunities for experienced Community Psychiatric Nurses to join our new North Kent Community Rehabilitation Service as the team's Senior Community Psychiatric Nurses. The Community Rehabilitation Service provides a flexible range of mental health interventions to help clients complex mental health needs to manage their mental health more effectively and remain in their own homes. The team works in close partnership with Social Care and housing providers to ensure each client has the necessary resources available to enable them to engage in their rehab journey. Across the Community Rehabilitation teams, the clinical team consists of Occupational Therapists, Psychologists, Nurses, and VCSE Workers. The successful applicants will care coordinate and provide rehabilitation nursing input to a defined caseload of individuals with complex mental health problems, they will deliver the therapeutic programme and provide support and supervision to other members of the staff team. The team delivers comprehensive, individualised treatment and recovery plans which include supporting clients to maintain their living environment, and encouraging and supporting them to have meaningful roles and purpose within their local communities. Individuals will be expected to ensure the team is able provide input between the hours of 08:00 and 20:00 according to the needs of the service, including working some evenings, weekends and public holidays. Main duties of the job The Senior Community Psychiatric Nurse is a key clinical leader within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience. The Psychiatric Nurse will be professionally accountable to the Executive Director of Nursing, and operationally accountable to the Service Manager The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
