This is an exciting opportunity for an enthusiastic, motivated individual to join our experienced Community Mental Health Teams.
This is a supportive, lively and busy multi-disciplinary team, which provides care co-ordination and recovery focused care for individuals with complex and challenging mental health needs. You will have an induction, regular supervision and on-the-job training alongside mandatory and statutory training.
You will need to be able to work autonomously in the community and use your own initiative under the guidance of qualified staff. You will work closely with the team's care co-ordinators to deliver effective and competent recovery focused care to adults with severe mental health needs, both in their own homes and in the local community.
We are looking to appoint a highly motivated, enthusiastic, flexible and resourceful individual who is committed to providing support and assistance to service users in their recovery. You will need experience of working with adults with mental health needs and an interest in working with service users in a community setting.
Main duties of the job
1. To provide physical and psychological care to service users in accordance with the specified plan of clinical care and in line with the recovery model.
2. To carry out duties as designated by the team leader.
3. To provide support to the person responsible for the clinical care of the service user.
4. To accurately record service user information.
5. To provide support to relatives/carers.

Person Specification
Qualifications
* Literacy and numeracy skills
* Previous experience in care
* NVQ 2/3 or equivalent
Experience
* Good communications skills
* Good organisational skills
* Knowledge of mental illness and the effects on service users and carers
* Persuasion, negotiation and engagement skills to undertake practical assistance
* Able to co-facilitate group work
