Are you innovative, like a challenge, think dynamically about person-centred care, and are passionate about older people who may experience a mental health problem? If so, we would love to hear from you.
At the Dementia and Older People's Mental Health Liaison service, no day is the same. Our service is based within the Royal Cornwall Hospital, Truro, but we outreach to our 8 local community hospitals and also provide community visits to support our patients and carers when they leave the hospital. As a band 6 practitioner, you will be involved in staff supervision, QI, audit, teaching, and the development of our service. The post of Psychiatric Liaison practitioner is a specialised mental health practitioner role specialising in older adult psychiatry liaison.
You will be required to have extensive knowledge and skills in caring for older people with mental health problems, risk assessment, and think outside of the box to ensure safe discharge out of the acute setting.
The post holder will utilise these clinical skills in ensuring the psychiatric liaison team provides high-quality psychosocial assessment care and treatment, facilitating a holistic approach to service delivery and discharge planning.
The term holistic in this context is of particular importance, as patients' physical care needs, in addition to psychological and social care needs, must all be carefully considered in the acute general hospital setting.
We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.
We are a people organisation, and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.
Over 4,000 people make up the Trust, including doctors, nurses, therapists, plus admin and support staff.
We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work, and a great partner.
Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays, lots of people choose to visit the area. This increases the numbers of people who use our services.
