Job summary The Tissue Viability team provides a vital service in the complex wound management of a variety of clinical conditions treated within the hospital and post discharge. You will also be assisting with the management of patients undergoing Plastic surgery and collaborating care pathways with both internal and external healthcare professionals to achieve best patient outcomes. As a Tissue Viability Nurse, you will work closely within the team and wider MDT, developing and improving patient care pathways whilst expanding your knowledge and expertise within the field. We are looking for someone who is seeking new challenges, is enthusiastic and motivated to inspire change. The position is part time (22.5 hours) over a Mon- Friday service. Main duties of the job The post holder will promote and provide patient centred care through: Using specialist knowledge and skills to assess the physical health status of both inpatient and outpatients within the specialty, making referrals to other agencies/disciplines as appropriate. Managing a defined caseload of patients working within agreed protocols and patient group directions, seeking advice, where necessary, from more senior practitioners. Undertaking nurse led clinic. Giving guidance on the management of patients through protocol/practice guidelines to support nurses' decision-making in clinical practice. Making changes to clinical management plans using evidence-based research and within protocols and clinical guidelines. Promoting best practice, collaborating with local and regional benchmarking groups to agree common standards and nursing and clinical guidelines. Presenting complex or sensitive information to multidisciplinary team, patients and relatives regarding care issues relating to the patient, including meeting the standards set for gaining informed consent. Contributing to professional led discharge by advising on transfer of care issues. Producing patient information leaflets specific to speciality as per Trust Policy.
