Are you a dynamic, confident and motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills and the ability to assess and lead on clinical decision making? If so, we would really like to hear from you.
This is an exciting role, working in a fast-paced environment within a well-established, multi-disciplinary team. The crisis and home treatment teams offer a high-quality assessment and home treatment service as an alternative to hospital admission for individuals experiencing acute mental health crises.
As a service, we are looking for individuals who are genuinely warm and engaging and focused on client/customer satisfaction.
Secondment opportunities/fixed team contracts for up to 12 months or permanent contracts are available. We also offer full-time and part-time working.

Responsibilities:
1. To provide first line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
2. To rapidly assess people with mental health problems who are vulnerable to hospital admission.
3. To provide specialist crisis assessments focusing on “hospital at home”.
4. To coordinate the Crisis and Home Treatment Team on a shift-by-shift basis, acting as Duty Worker to triage referrals and plan the shift's activities accordingly.
5. To participate in team MDMs.
