Job summary
The LTP introducedIntegrated Stroke Delivery Networks (ISDNs) in all areas of England, bringing people and organisations together to deliver the best possible care for their population. The overarching aim of an ISDNs is to enhance the quality of stroke care, by improving clinical outcomes, patient experience and patient safety. The ISDNs do this by bringing key stakeholders together, to facilitate a collaborative approach to service improvement of the whole stroke pathway ensuring a patient centred, evidence-based approach to delivering transformational change.
In spring 2021, underpinned by the National Stroke Service Model, providers and partner organisations from the following systems BLMK, H&WE and M&SE came together to form the East of England (EoE) South ISDN. At the same time the following systems N&W, C&P and H&WE came together to form the EoE North ISDN.
The EoE ISDNs are now well established with a proven track record to deliver system wide transformation and improvements in stroke care.
To support current EoE ISDN priorities and to support the SQuIRe (Stroke Quality improvement in rehabilitation) programme, the EoE are welcoming applications for a programme manager for EoE South ISDN.
Working collaboratively with the regional CVDR Clinical Network leads, SQuIRe lead and ISDN clinical leads, the programme manager will lead and support improvement in stroke services across the East of England.
Main duties of the job
Underpinned by dynamic transformational skills based on demonstrable experience of delivering complex transformation projects, the programme manager will lead projects and transformation initiatives at both an ISDN level and across the regional footprint. This work will include but will not be limited to:
1. Engaging and communicating with a wide variety of key stakeholders to promote sustainable and equitable stroke services of the highest quality possible.
2. To act as a conduit for information and initiatives between national, regional and local systems.
3. Ensuring full engagement with the Sentinel Stroke National Audit Programme (SSNAP), monitoring network performance and recommending support.
4. Acting as a champion for stroke patients, carers and their interests.
5. Supporting effective stroke care pathways across the ISDNs assisting with clinical collaboration and coordination between all stakeholders including the third sector supporting the monitoring of consistent, high quality stroke care through the delivery of national stroke models and the national guidance for stroke.
6. Support the ICSs to develop a strategic approach to improving local stroke pathways.
7. Manage effective cross-boundary and border patient flows with neighbouring ISDNs.
8. Support the delivery of the optimal configuration of stroke services within a given geography.
