We require a friendly, motivated, hardworking committed healthcare assistant to join our outpatient team. You need to demonstrate excellent communication and organisational skills; you are required to be self-motivated with initiative. You will work independently within the multidisciplinary team always ensuring the patients' needs are met. You will be working under the supervision of a registered health professional.

Key Responsibilities:
1. Participate in the assessment, planning delivery and evaluation of patient care, following agreed plans, policies, procedures, and clinical guidelines, independently carrying out tasks and activities relevant for the job role/place of work, which have been agreed and assessed as competent in practice.
2. Work collaboratively within the multidisciplinary team/agencies to ensure that patient needs are met.
3. Involve patients, relatives/carers in the delivery of care, providing reassurance and support to assist their understanding and co-operation.
4. Prepare the clinical area for safe practice by ensuring that cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwards.
5. There may be times when you are required to support working at the Satellite unit and Victoria Infirmary, Northwich.
