Responsibilities
As a Team Coordinator you will:
1. Deliver One-to-Ones for Support workers.
2. Complete Return to Work interviews for Support Workers.
3. Chair some Micro-Team Meetings where required.
4. Carry out observations of social care practice, including medication assessments.
5. Take a lead role in Quality Assurance audits and other management reporting duties where required.
6. Contribute to the writing of care plans, including assessment and risk management plans.
7. Support On Call once a month by being available to cover last minute Support Worker absence for which you will be paid £150 retainer plus your hourly rate for any support delivered. (On call starts at 5pm on a Fri to 9am the following Mon)
8. Support roster writing and making amendments to roster where required.
9. Work 70% of your time providing direct support with 30% being management duties.
10. Be open to direction from your Locality Manager.
11. Build and maintain professional working relationships with your teams and colleagues.
Essential Requirements Include:
1. Hold a Full driving licence and have access to a reliable car with business insurance.
2. Have the ability/be willing to travel across Somerset and bordering counties.
3. Hold at least two years’ experience of working with autistic people and people with a learning disability or complex needs.
4. Hold a Level 3 in Health and Social Care or be committed to achieving this.
5. Ability to deal with challenging situations in a calm and effective manner, managing risk.
6. Creative and innovative approach to solution finding.
7. The ability to communicate effectively with internal and external stakeholders.
8. The ability to work alone, on own initiative without direct supervision.

Duties Include:
1. Deliver services based on the expressed wishes, cultural needs, and support plans of those who use our services.
2. Assist and encourage people to make choices and life decisions in line with the Mental Capacity Act.
3. Facilitate the accessing of local social and community facilities.
4. Assist and encourage people to engage in everyday home, social, leisure, and work activities based on their personal choices.
5. Support people to manage their own homes, including supporting people to manage their own tenancies and providing safe support and guidance where necessary.
6. Provide direct personal care and support in line with the individual's support plan, which may include toileting and bathing if required.
7. Work as a positive, constructive, and enthusiastic team member.
8. Support those who use our services to take prescribed medication as directed by appropriately qualified and trained health professionals.
9. Contribute toward the personal development of those who use our services through promoting, enabling, and developing communication. Provide stimulation through the planning and delivery of programmed activities.
10. Follow confidential processes for the appropriate collecting, handling, and storage of information. Maintain any records pertaining to those who use our services. Effectively communicate information to those who have a need for such information. Contribute toward statistical returns and secure financial record keeping, including the personal finances of each person served.
11. Contribute toward You First’s Life Design assessment process and the development of Person-Centred Support Plans in conjunction with team colleagues, the person we support, their family, and other key professionals.
12. Assist and advise those who use our services with basic personal finance management and local shopping activities as directed and when required.
13. Support team colleagues and respond to the immediate health and well-being needs of those who use our services by ensuring timely support and professional assistance is obtained from team colleagues and others when required.
14. Maintain day-to-day contact with a range of health and social care professionals and other agencies to pass and receive information relating to those who use our services. Plan, contribute toward, and attend meetings as a member of the care support team.
15. Take responsibility and joint ownership for own personal and professional development. Access training as and when required and contribute toward effective and innovative care.
16. Act as a role model and mentor to new staff. Undertake waking nights and sleep-ins as and when required under the direction of the management team.
Salary: £14.75 per hour
