To develop a rapport based upon respect and humanity that builds the basis of a professional relationship. To consistently support service users in engagement with their agreed Care Plan and in accessing appropriate services. To positively promote independent living of service users within the community by supporting and participating in daily living activities. To provide regular and practical support to people with a learning disability and autism and their carers in developing and managing dignity and independence. To provide support and participate with service users in undertaking daily living and living ordinary lives. To help service users gain access to community resources. To provide information on health promotion. To help to identify early signs of relapse by monitoring the user of services progress, level of functioning and mental state and alert the appropriate staff involved in the clients care. To liaise regularly with key workers and other people involved in providing care. To attend and actively participate in care planning and team meetings. To maintain adequate records as required by existing procedures, entering appropriate details on the user of services case notes as necessary. To participate in the review and development of the service, promoting service user involvement in the process. To attend and actively participate in training and development sessions and supervision.
