The Princess Alexandra Hospital NHS Trust
The Paediatric Diabetes department is looking for an enthusiastic and dynamic individual who has a genuine interest in caring for children with diabetes to join the team on a fixed-term basis. While experience in this area would be an advantage, it is not essential as we will offer tailor-made support and training. If you do not fully meet the criteria for this post, we would consider offering a Band 6 developmental role instead to the right candidate.
We are a well-established service consisting of two Consultants, two Paediatric Diabetes specialist nurses, a Paediatric Specialist Dietitian, and dedicated clerical support. This is primarily a community-based role with aspects of acute hospital care and is based in the hospital.
You will provide specialist paediatric diabetes care management for all children and young people across West Essex and specific areas within East & North Hertfordshire in collaboration with the wider team and multi-disciplinary teams.
We are looking for someone who is enthusiastic and keen to learn, organised, and able to work independently as well as part of a multidisciplinary team.
We are a very friendly, hardworking, and committed team, and informal visits are encouraged.
Main duties of the job
* To support the provision of holistic specialist diabetes nursing care in the hospital, home, or community setting with the support of existing Band 7 Paediatric Diabetes Specialist Nurses/Paediatric Specialist diabetes dietitian and the wider children's diabetes MDT.
* Provide support and advice to children, young people, and their families from diagnosis through to discharge/transition to adult services (0-19yrs).
* Support the implementation of the Paediatric Diabetes Best Practice Tariff and NPDA with the wider Children's Diabetes MDT team.
* Provide teaching and education to other colleagues within health and from other agencies.
* Support and deliver a formal teaching programme for children, young people, and their families with the wider multidisciplinary team.
* To ensure that all children and young people have individual care plans in conjunction with the multi-disciplinary team.
* To work towards acting as a source of specialist advice for children, young people, and families on the nursing care and management of diabetes, including the provision of basic dietary advice and the management of acute complications.
* Work closely across the primary and secondary care providers to establish and maintain best practice for transition into adult diabetes service.
* Undertake relevant training and professional development to enhance and develop within the role.
