A Vacancy at Greater Manchester Mental Health NHS Foundation Trust.
The purpose of this post is to develop the interface between Community (CMHT and Living Well_ and Acute (Inpatients and Urgent care) services.
Based at Cromwell House but available to work across Salford Community Services as required.
Will also be required to go to Inpatient units as required trust wide.
The post holder will:
Maintain oversight of all community patients (CMHT and Living Well) in inpatient and urgent care settings
Maintain oversight of patients identified as requiring admission
Ensure appropriate community care following acute care (Inpatient and Urgent Care) episode
Complete initial assessments for CMHT in urgent care/ inpatients
Complete commissioning work for people with no CC or a non-social work CC
Develop extensive knowledge of local providers
Liaise with MDTs in Inpatients/ Community and Urgent Care
Lead discharge planning for community patients
Act as CC for up to 4 weeks – up to 5 patients to facilitate discharge planning and ensure smooth transition to community
Support Care Coordinators to be active participants in acute care
