Job summary We are seeking an enthusiastic and skilled individual to join our multidisciplinary team, which includes a Consultant Practitioner, Advanced Clinical Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists, Paramedic Practitioners, and Health Care Support Workers. The team provides an urgent response service to patients experiencing a health crisis, with the goal of supporting them at home rather than requiring hospital admission. The post holder will play a vital role in the triage and coordination of referrals, ensuring that patients receive timely and appropriate care. This role involves processing incoming telephone referrals, assessing acuity, and coordinating team responses. The referrals come from ambulance crews, emergency departments, NHS 111, GPs, community and specialist therapy/nursing teams, and self-referrals. Using professional judgment and critical thinking, the post holder will determine whether to accept referrals, allocate resources accordingly, or redirect cases to appropriate services. This position offers a hybrid working model, allowing for both home-based and office-based work. Main duties of the job Clinical Triage and Referral Management Process all incoming telephone referrals and assess patient acuity to determine appropriate response. Make clinical decisions based on Urgent Community Response (UCR) and Hospital Home (HH) Virtual Ward (VW) guidance. Case manage referrals to ensure timely and effective service delivery. Escalate urgent cases or redirect referrals to appropriate professionals when necessary. Coordination and Communication Work collaboratively within a multidisciplinary team to provide patient-centered care. Allocate visits and coordinate resources within the clinical triage team. Liaise with referrers, including ambulance services, emergency departments, GPs, NHS 111, and community teams. Maintain accurate and timely documentation of referral decisions and patient interactions. Professional Standards and Teamwork Uphold high professional standards in all clinical interactions. Contribute to service improvements and help develop best practices in triage and urgent response care. Work in a fast-paced environment, making critical decisions to support patients in crisis.
