We are looking for an enthusiastic and adaptable Urgent Care Service Assessor to join our strong and progressive team in the Folkestone area of South Kent Coast.
You will share the CARE values of the trust and work within a close-knit group of people who bring the feeling of community to the team through their hard work and dedication to nursing and to the people of the area.

Folkestone is made up of a diverse patient group with complex needs and by working in a community-based way we can ensure that services are targeted and patient-focused, and we are proactive in our input and response.
You will play a vital role in our multidisciplinary services; supporting patients with complex needs in the community. You’ll have the freedom to plan care in your own way, with time to develop your assessment and treatment skills with the support of senior staff.
Our aim is to build on collaborations so that we can continue to build a positive patient experience. As a short-term service, the predominant part of your role with be assessing, planning and reviewing care needs, liaising with other team members to ensure safe, high quality care.
You will be required to drive between patient homes and work sites and will need to provide your own transport to achieve this.
