Join our Integrated Hospital Discharge Service, where you'll take on a vital leadership role, supporting safe and timely discharges for adults from both Acute and Community hospitals. As an Advanced Practitioner, youll provide practice and professional leadership, guiding Social Workers and social care staff to deliver exceptional outcomes. In this role, you will provide statutory support in acute and community hospitals, to adults with care and support needs, as well as their carers and families. You will act as a lead for adult social care about the hospital discharge process, working as part of a Multi-Disciplinary Team ensuring seamless transitions of care while acting as a crucial link between health partners, community teams, and social care services.
