St Margaret’s Hospice Care has a fantastic opportunity for a Social Work Assistant to join our Supportive Care team. You will join us on a permanent basis and in return, you will receive a salary from £26,530 (pro rata for part time hours). This role is based in Taunton and Yeovil and involves travel throughout Somerset to undertake community work, home visits, and meetings. This role has a working pattern of 30 – 37.5 hours per week. The post is hospice based (Taunton and Yeovil) with the possibility of working at home one or two days a week. What can you expect to be doing? You will be joining an established and super supportive team with a collaborative working culture. As our Social Work Assistant, you will hold an autonomous caseload of patients with a palliative, life limiting diagnosis. You will also support patients’ family and carers. Under the guidance of the supportive care lead and social worker, your day-to-day work will include: Working with the clinical teams to ensure the social and supportive care needs of patients and families are assessed and where possible met. Providing safeguarding advice and guidance to clinical staff, patients, families, and carers, ensuring the welfare of vulnerable adults and children. Supporting with safeguarding concerns and promoting best practices. Making appropriate referrals to the multi-disciplinary team and a wide range of community-based services. Supporting with benefit checks. Collaborating with multidisciplinary teams to deliver compassionate, patient-centred care. What will you be good at? You will need: An NVQ Level 3 or (equivalent) in related care work Experience working in a health or social care environment Knowledge of the Care Act 2014 Ability to communicate effectively with patients, families, and healthcare professionals Ability to work collaboratively in a multidisciplinary team environment Flexible, able to adapt to changing or conflicting priorities. Additional Requirements: Full driving license required as travel is essential as the post is over both Taunton and Yeovil hospice sites. Ability to travel locally to attend meetings and home visits as necessary.
