Main duties of the job
Using GP systems (Emis and S1), you will support practice sites with medicines-related tasks, which may include but are not limited to:
* Supporting patient requests for acute and repeat meds
* Structured medication reviews
* Safety audits

Job responsibilities
You will work as part of a multi-disciplinary team in an onsite role, collaborating with colleagues to help resolve medication-related issues, improve patient outcomes, and ensure better access to healthcare.
You will support practice staff with prescription and medication queries, help support the repeat prescription system, deal with acute prescription requests, and conduct medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.
You will support prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.

Experience
* Primary care and/or GP practice experience is essential
* Experience using clinical systems Emis / System1
Qualifications
* Completed CPPE Pathway / currently on CPPE pathway
