Job summary This is an exciting new opportunity to join our team at The Orchards which comprises of a 20 bedded Acute Ward and 10 bedded PICU for both male and female adults. We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Hampshire and Isle of Wight Healthcare NHS Foundation Trust has brought the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities. To carry out comprehensive physical health assessments for service users, identify and deliver relevant interventions as needed, such smoking cessation, exercise groups, diet advice, and alcohol advice, as per NICE guideline standards. The assessments may take place within inpatient wards or at another trust site. The role may require independent and lone working at times. The role will develop to include delivery of educational groups regarding good physical health regimes. Main duties of the job To carry out comprehensive physical health assessments for mental health service users. To support the ongoing delivery of the NICE Quality Standards ensuring service users have received a comprehensive physical health check, which includes the following measures: smoking status, alcohol intake, substance misuse, BMI or waist circumference, blood pressure, blood glucose, and cholesterol. To support the ongoing delivery of the NICE Quality Standards ensuring service users are offered relevant interventions for their physical health including, smoking cessation, harmful alcohol use, substance misuse, weight gain or obesity, hypertension, diabetes/high risk of Diabetes, Dyslipidaemia. Provide information on improving health and wellbeing and signposting to other agencies as appropriate.
