Who You Are Possess a DipSW or CQSW or equivalent qualification and are HCPC Registered. Have proven experience and knowledge in mental health work within the statutory framework. Skilled in multidisciplinary team work and care management. Possess excellent written and verbal communication and engagement skills. Possession of an AMHP qualification and a driving license is desirable. What the Job Involves Provide assessments, care management, and social work services under relevant legislation and Council policies. Collaborate with the NHS Mental Health Trust’s multidisciplinary Community Mental Health Teams. Coordinate care plans and packages while ensuring compliance with financial regulations and local policies. Serve as a care coordinator and manage the requirements of the care program approach. Participate in multidisciplinary team duties, including safeguarding adult inquiries and the borough's AMHP rota. Stay informed about current legislation and policies related to mental health and vulnerable persons. Foster professional relationships with mental health staff and other professionals across various sectors. Promote equality of opportunity and adhere to Health and Safety policies in service delivery.
