A new and exciting opportunity has arisen for a Clinical Pharmacist to join our team here at Heath view Medical Practice.
We are a friendly and enthusiastic team with 6 GP Partners, Physician Associates, Nurses and HCAs.

Main duties of the job
The successful candidate will have direct contact with patients, taking medical histories, carrying out physical examinations, updating records, making diagnoses, analysing test results, undertaking home visits, and delivering treatment. Working to a professional code of ethics, you will be able to demonstrate flexibility in the role and have a caring attitude towards patients.

The Practice is constantly looking at improving ways to provide better and more efficient patient care. The post holder will be well supported in this Primary care role by an accountable GP(s). You will be expected to work across our 3 sites alongside both our GP and nursing staff to meet the needs of patients by providing direct clinical care using clinical guidelines.
Job responsibilities
1. Participate in all treatment and preventative healthcare services in the practice as delegated and agreed by the supervising GP.
2. Provide direct clinical care to patients using established clinical guidelines.
3. Recommend and explain appropriate diagnostic tests and treatment.
4. Discuss patients with problems needing referral to secondary care with the supervising GP before making such referral.
5. Ensure evidenced based care is delivered at the highest standards ensuring delivery of high quality patient care.
6. Establish and maintain good liaison with other surgeries and agencies, including secondary care.
7. Undertake face to face and telephone consultations for emergency or routine problems, as determined by the practice, including management of patients with long-term conditions.
8. Contribute positively to the effectiveness and efficiency of the team and work colleagues.
Person Specification
Qualifications
* Registered with the GPHC
* Educated to degree/masters level in Pharmacy, MPharm/BPharm (or equivalent)
* Clinical Diploma
* Independent prescribing or working towards/intent of gaining an independent prescribing qualification.
