This is an exciting opportunity to join our progressive team as a clinical pharmacist working across Heywood primary care network (PCN), Greater Manchester.
We are looking for an experienced and highly motivated pharmacist to join our team of PCN based pharmacists and technicians and will have access to peer support and development opportunities as part of our MDT.
This is an on-site position for 4 days (30 hours) per week. Candidates should hold an independent prescribing qualification and CPPE PCPEP trained.

Job Responsibilities:
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face-to-face structured medication reviews, manage long-term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both the public health and social care needs of patients in the 5 GP practices that make up Heywood PCN.
The post holder will perform face-to-face medication reviews of patients with polypharmacy, especially for older people, people in residential care homes, and those with multiple co-morbidities and long-term conditions. The post holder will provide leadership on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework. This role is pivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice.
Person Specification/Qualifications
Essential
* Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
* Qualified or enrolled on CPPE Clinical Pharmacist Training pathway
* Hold an independent prescribing qualification

* Previous general practice experience
* Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role
* Strong clinical leadership, communication and interpersonal skills
* Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role
Desirable
* Experience in supporting common acute and long-term conditions
* Experience of working within a care home setting in the capacity of providing patient-centred medicines optimisation support
* Knowledge and understanding of Primary Care Network arrangements
* Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support
