We are a small team based in Kent and Canterbury Hospital where we work closely together to offer clinical support services to each hospitals within the trust. Pharmacy Homecare is a service that delivers ongoing medicine supplies, and, where necessary, associated care, initiated by the hospital prescriber, direct to patient's home. The Pharmacy Homecare team are an intermediary between the clinical team and the Homecare company and play a critical role in patients receiving their medicines. Pharmacy Homecare have to ensure the right drug, in the right form is ordered and delivered at the right time to the patient. Within our Homecare team, we are looking for two part-time enthusiastic members of staff to join us. Staff member one: 22.5 hours per week, staff member two: 15 hours per week. As a team we are working hard to ensure we put patients at the heart of everything we do. We are on an exciting journey, ever changing, improving and gaining new knowledge and expertise to develop as the East Kent Pharmacy Homecare Team. To liaise and co-ordinate good communication between homecare companies, patients, carers, Trust pharmacy and medical staff.
