Job summary We have an exciting opportunity for a Band 5 Nurse to join our Living Well After Stroke Team as part of our Integrated Community Stroke Service Team here in Somerset. The role will support our countywide specialist stroke service along with branching out to somerset wide tertiary providers of health and wellbeing. The successful applicant will manage a clinical caseload of stroke patients, and lead and mentor band 4 nursing and support workers. You will work alongside a multidisciplinary team, including Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, and Rehabilitation Assistants. As well as forging links outside the NHS with our VCS partners and wider community support services. Experience in stroke care, mentoring, and clinical supervision is essential. You will possess excellent communication skills and be required to write proficient treatment summaries of the stroke specialist care provided. You will also support the development of the stroke nursing and LWAS workforce, ensuring high-quality care across the county. The successful applicant will be supported by the Band 6 Integrated Community Stroke Nurse and Band 7 LWAS Lead and have access to ongoing development opportunities.
