To adhere to the Principles of Compassion in Practice, and ensure that the culture of the service is enabling for patients independence. To carry out assessment of patients as an autonomous practitioner, using clinical reasoning skills and knowledge of evidence based practice to formulate appropriate care/treatment plans, utilising a range of treatment skills and options. To use assessment findings to design patient care nad rehabilitation programmes which may be implemented by other members of the multidisciplinary team. To maximise the functional ability and occupational performance of an individual by developing realistic treatment plans and goals on admission and discharge, and through discussion with the patient, senior staff and the multidisciplinary team, develop an awareness of patient prognosis. To delegate appropriate tasks effectively to Therapy/Rehab assistants. To appreciate the need for patient privacy and dignity at all times. To reassess patients in order to progress and/or modify care plans appropriately and effectively. To show respect to patients own beliefs, values and differing cultural needs. To manage patient discharges effectively To motivate and engage patients and, where appropriate their families/carers in the patients rehabilitation. To participate in the education of other staff, patients, families and carers as appropriate. To consider the needs of carers to ensure necessary support systems are in place and to involve them in their goals and needs To participate in collecting standardised measures of outcome as required by the service. To be aware of all issues relating to Clinical Governance and to meet standards as required. To maintain clinical and care records for own interventions in line with organisational and legal requirements. To manage risk within own caseload at all times, completing risk assessments and ensuring knowledge of and compliance with organisational and professional guidelines. To carry out a full range Occupational Therapy assessment and interventions appropriate to the caseload.
