Job summary We are seeking an enthusiastic Advanced Nurse, Paramedic, OT or Physiotherapist to expand our home visiting team. A clinician with a special interest in frailty and older people, supporting delivery of same day care in the community. Main duties of the job Main duties a) Undertake same day urgent visits, prioritising in order of clinical need, acknowledging this can change at short notice due to service demand. b) Undertake comprehensive geriatric assessments including medication reviews and advanced care planning where appropriate. Take an accurate history, undertake suitable systems examination including observations and other point of care tests as appropriate. Diagnose and manage acute and chronic conditions. Utilise and refer to community services where appropriate, ensuring patient safety throughout. Communicate findings and management plan with patient/relatives and referring clinician. Provide safety netting/ worsening advice for patients and relatives. Ensure comprehensive, timely and accurate documentation.
