Position Overview: We are seeking dedicated and experienced Home Health Physical and Occupational Therapists to join our team. You will provide individualized rehabilitation services to patients in the comfort of their homes. You will be responsible for evaluating, designing, and implementing customized treatment plans to help patients achieve their goals and improve their physical well-being.
Key Responsibilities:
* Conduct comprehensive assessments of patients' physical abilities and develop personalized treatment plans.
* Provide physical therapy interventions for patients in their home settings, including therapeutic exercises, manual therapy, and mobility training.
* Collaborate with other healthcare professionals, including nurses, doctors, and caregivers, to provide holistic patient care.
* Educate patients and families on physical therapy techniques, safety practices, and exercises to promote long-term wellness.
* Monitor patient progress, document treatments, and adjust treatment plans as necessary.
* Adhere to all healthcare regulations, policies, and procedures.
* Maintain accurate and timely records of patient visits, treatments, and outcomes.
* Ensure all home therapy services are delivered with empathy, professionalism, and respect.
Qualifications:
* Degree in Physical Therapy from an accredited program.
* Active state licensure as a Physical or Occupational Therapist.
* Previous experience in home health care preferred.
* Strong knowledge of physical therapy techniques, treatment modalities, and patient assessment.
* Excellent communication and interpersonal skills.
* Ability to work independently and manage a caseload effectively.
* Current CPR and First Aid certification.
* Valid driver’s license and reliable transportation to travel to patient homes.
