Pathway Co-ordinator and Operational Support

Take the lead in providing operational business support for the Service Managers in Neurology and Stroke.
Provide full and comprehensive administrative support to the operational team to enable the Service Managers to meet the expectations of key performance indicators.
Provide line management to the administration team working within the Medical Day Unit.
Improve systems and processes in conjunction with the Clinical Pathway Co-Ordinator on the Medical Day Unit.
Main Duties of the Job
Take responsibility for the co-ordination of the patient's pathway to ensure that the patient's journey is managed efficiently, smoothly, and in accordance with targets and agreed timescales.
Bring appointments forward for patients at risk of breaching RTT targets and chase test results with the appropriate departments such as Radiology, Nuclear Medicine, and Cardiology.
Report on long waiters, undated patients, and those dated over 18 weeks awaiting diagnostics/results.
Liaise with different stakeholders including patients, GPs, and clinical teams.
Support the operational team with aspects of service development.
Admin Management
Be responsible for the day-to-day line management of the Medical Day Unit administrative team, being the first point of contact for all areas of management including performance and conduct.
Daily validation of all PTLs to ensure patient pathways are of good quality and comply with Access Policy standards.
Work collaboratively with the clinical teams on each site to plan activity, making full use of all available capacity and developing plans to address capacity issues. Ensure agreed actions are delivered and staff are accountable for delivering and have appropriate knowledge, skills, and training to achieve targets.

Skills and Experience
* Worked within the NHS for at least 2 years.
* Ability to set up systems and procedures to support the service.
* Managing patient pathways and ensuring these meet RTT targets and deadlines.
* Experience of working within a team.
* Able to use own initiative managing a complex workload with competing demands.
* Well organised and able to prioritise own workload and that of others.
* Ability to work under pressure and problem solve, meeting demanding deadlines.
* Experience of supervision of staff.
* Able to multi-task in a fast-paced environment.
Qualifications
* Computer literate including Word and Excel; range of NHS computerised systems.
* Educated to GCSE level.
* Evidence of IT skills.
