The Greenfield Centre is looking to recruit a full-time Mental Health Nurse.
The post holder is responsible for the delivery of care and for the supervision of others who are carrying out all aspects of the caring process. The post-holder will assist in setting and maintaining standards of care, participate in maintaining effective management of the team, and maintain effective integration with the multi-disciplinary team while assisting with the facilitation of a collaborative approach to patient-focused care.
Main duties of the job
1. Work within the care team, triaging referrals, undertaking holistic initial assessments, and supporting with duty professional cover.
2. Act as a Change Agent and actively seek to develop practice.
3. Develop relationships and strengthen links, liaising between the different care providers.

Job responsibilities
1. Participate in the triage of new referrals.
2. Ensure that appropriate interventions are planned and delivered according to assessed need.
3. Promote a positive service user relationship and engage with carers/relatives as appropriate.
4. Document the plans of care and ensure that plans remain current, adhering to standards for record-keeping.
Person Specification
Qualifications
* Evidence of professional updating and development in the delivery of services to people with mental illness.
* Professional registration - Nursing.
Experience
* Experience of working therapeutically in community settings.
* Experience of working with a range of statutory and non-statutory agencies.
Knowledge
* Awareness of harassment, social exclusion issues, and the effects of personal and institutional discrimination.
* Skills in assessing, planning, and evaluating mental health care/treatment interventions.
