AO is a tertiary service who takes all referrals from Secondary care services. We are looking for an enthusiastic, committed, hardworking individual to join the team. You will work under the guidance of the Senior Practitioner as a member of the MDT providing community services to adults with a enduring mental health problems in their own home. You will hold a caseload of people with enduring mental Health difficulties and be responsible for the assessment, planning and evaluation of packages of care. You will require excellent communication skills and be able to communicate with a wide range of people both verbally and in written format. Job Description Summary (Main duties and responsibilities) The successful candidate will be a Lead professional for a allocated caseload of patients following the principles of the Care Co-ordination policy, being an integral member of the MDT, attending and contributing to MDT discussions. To maintain accurate record record keeping in line with NMC guidelines and Trust policies and procedures, co-ordinating and prioritise your workload on a day to day basis The post holder will act in a professional manner and adhere to the NMC Code of Professional Conduct at all times, maintaining continuing professional development. Actively participating in the supervision and appraisal processes, adhering to Trust policies, procedures and clinical guidelines.
