At Sandwell and West Birmingham NHS trust we are very proud of our specialist palliative care and End of life care Co-ordination Hub. The CQC awarded us excellent for our service in 2017. The team consists of specialist Palliative consultants, palliative care clinical nurse specialists for the Acute and community, end of life care facilitators, an Urgent response team and Specialist Macmillan Occupational Therapists & Physiotherapist. The Connected Palliative Care Hub provides a 24/7 service to patients who are in hospital and the community in Sandwell and West Birmingham. The Macmillan Therapy Team offers services to palliative patients in the Acute Hospital, Day Hospice and in their own home.
Main duties of the job
The successful candidate will be able to navigate the faced paced nature of facilitating Fast track discharges home for patients within the Acute Hospital setting, where home is their preferred place of care. Carrying out Holistic Needs Assessments on palliative patients establishing client centred goals that patients wish to achieve along their journey taking into consideration limitations surrounding their disease trajectory. Using therapeutic activities to provide advice and techniques that empower patients' self-management of symptomatic relief. Supporting these patients with advanced care planning and promoting quality of life up to and until end of life.

Job responsibilities
This unique team set up allows the post holder to develop skills within palliative care across Acute, Community and Day Hospice settings. This well-established specialist palliative care team will impart their experience and knowledge to support this new post holder. You must be a registered Occupational Therapist with core skills, developed experience and clinical knowledge around palliative symptom control and end of life care. You will have access to structured clinical supervision as professional development is a high priority along with the expectation that you will supervise and support colleagues, junior staff and students. You must be a car owner/driver and be able to organize and prioritise your caseload and implement patient plans of care independently. The successful candidate will be an effective communicator with broad clinical experience, who is able to demonstrate personal and emotional resilience, the ability to adapt to changing pressures, a caring approach and a commitment to delivering the best possible quality care.

Experience
* Post OT qualification experience in a variety of clinical areas
* Experience of managing complex patients in the community
* Experience in risk assessment
* Experience of supervising and delegating work to support staff
* Experience of using communication skills in challenging circumstances
* Experience of forging excellent relationships across professional boundaries
* Experience of caring for patients at the end of life / with specialist palliative care needs
* Experience of transforming practice to improve patient outcomes
* Experience of working with professionals from health and other sectors including social care and the 3rd sector
* Participation in clinical /service audits
* Experience and knowledge on a range of equipment, beyond that available on the usual catalogue
* Contribution to development of policies
Communication
* Ability to effectively present information in both written and verbal format
* Clear clinical reasoning demonstrated within Holistic Assessment
* Ability to communicate where there are barriers to understanding and show effective listening skills
* Completed Advanced Communications course or other palliative care specific training
