Providing full GP service to practice population including (but not limited to); assessing the healthcare needs of patients, screening patients for disease risk factors and early signs of illness, providing counselling and health education and accurately referring or signposting to secondary care where appropriate. Completing daily tasks accurately and in a timely manner. These can include and are not limited to actioning results and clinical correspondence, signing repeat prescriptions, dealing with queries and completing reports.
Supporting patients in the management of chronic diseases, conducting periodic reviews where appropriate and correctly coding patient records for the purpose of QOF.
Working collaboratively with practice admin team to ensure referrals, tests and appointments requests are correctly completed and communicated. Using best clinical practice in terms of prescribing. Delivery of enhanced service, utilising any speciality where appropriate. Embracing technology and utilising clinical system (EMIS) to full potential to create efficiencies and improve patient outcomes.
