As a Senior Practitioner, your role will include delivery of a variety of functions across the CHC service, including: Assessment Reviews Care Planning Case management You will also have the opportunity to work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more. You will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively. NHS Sussex is strongly committed to your development, and through a structured 1:1 and appraisal system, will work with you to recognise and create a plan to meet your development needs. The role will require travel on occasions, to carry out assessments and reviews. Access to transport is therefore necessary, be it personal or public. All CHC staff are expected to work flexibly, when required, to meet the needs of the service.
