Sunderland City Council delivers a community based Weight Management programme aimed at improving the overall health and wellbeing of residents by supporting them to achieve a healthy weight, increase activity levels and improve a range of health indicators.andnbsp;

We are seeking to recruit 2 x Active Sunderland Weight Management Support Officers who are passionate about making a difference.andnbsp; Candidates should have a sound understanding of nutrition and weight management, be able to deliver low intensity group exercise classes to residents who are currently inactive, and have experience of working directly with residents to encourage and implement behaviour change.andnbsp; andnbsp;andnbsp;
Candidates will work independently from one of our community venues and will be responsible for all aspects of the programme at their site.andnbsp; The role requires candidates to:
Deliver one to one appointments up to 26 weeks
Support residents to make improvements across a range of health indicators including healthy weight, sleep quality, physical inactivity and mental health
Work with local health partners to promote the service to eligible residents
Liaise directly with eligible residents to book all appointments
Record and input accurate health data into an online database after each appointment to evidence progress and improvement for each individual resident
Plan and deliver weekly drop-in sessions, providing advice and guidance on topics such as the Eatwell guide, portion sizes, physical activity and sleep health
Deliver low intensity group based physical activities to residents who are currently inactive as part of the drop-in session
Produce a range of health content including videos and presentations to be shared with residents on the programmeandnbsp;
Candidates should have experience of delivering a community-focused health programme with evidence of good outcomes and be confident in dealing with a range of residents who often have complex health needs.

Whilst this role can be demanding, it is also incredibly rewarding as you will help residents improve a range of health indicators making a real difference to their lives.
This post is exempt from the rehabilitation of Offenders Act 1974 and therefore will be subject to a DBS check from the Disclosure and Barring Service.
