What you will do:
1. Provide professional social work support to adults with care and support needs, undertaking a range of statutory duties in accordance with relevant legislation.
2. Assess the needs of individuals and their carers, planning, implementing, reviewing, and evaluating outcomes to ensure high-quality support.
3. Promote principles of choice and control, empowering individuals to make informed decisions about their care and support.
4. Adopt a strengths-based approach to help individuals build support networks and achieve their identified goals.
5. Manage an autonomous caseload while embracing a multi-agency approach, collaborating effectively with health and care sector colleagues.
6. Deliver advice and support to other adult care practitioners, guiding them in their work with clients.
7. You will practice in accordance with Social Work England, Professional Capabilities Framework, and Knowledge and Skills Statement for Social Workers in Adult Services.
8. You will manage workloads in line with your qualifications and experience while enabling self-directed support for individuals and their carers.
9. Undertake risk assessments, ensuring adherence to legal principles of safeguarding and positive risk-taking. Foster innovative use of community resources and technology in your practice.
10. Contribute to the development and supervision of social care workers and undertake roles in more specialised areas, such as Approved Mental Health Professional or Best Interest Assessor, as needed.
