Case Worker
Location: Rotherham
Hours: Full time, 39hrs per week, Monday to Friday
Salary: £25,115.00
Overview:
Target Housing is looking for a Case Worker within South Yorkshire to work within our Sheffield complex needs project. The purpose of this role is to provide high quality holistic support to our clients. As a Case Worker, you will lead on support for your allocated caseload of clients, coordinating with others internally and externally as required. You will work with your clients to enable them to develop their knowledge, skills, and confidence so that they can increasingly make their own decisions and take more responsibility for their own lives and communities.
Job Role:
You will provide high quality housing management and support services to clients in order to assist them in sustaining their tenancies, achieving their potential, and working towards independent living. You will be working with clients on a 1:1 basis and will oversee and co-ordinate support for allocated clients, including completing risk assessments, support plans, and safety plans. You will also monitor clients’ compliance with their occupancy agreements and act to resolve non-compliance issues.
Person Specification:
* Experience of supporting people with multiple complex needs including hard to reach clients who struggle to engage.
* Experience in building professional relationships to enable development.
* Broad understanding across a range of relevant issues.
* Relevant qualifications and/or knowledge in specialist areas such as Substance Misuse, Dual Diagnosis, Domestic Abuse, brain injury, Mental Health, and Welfare benefits is desirable but training can be provided to the right candidate.
* Experience in lone working with vulnerable clients.
* Ability to record client notes on an I.T system and verbally communicate handover to colleagues.
* Understand the principles of PIE (Psychologically Informed Environments) and TIP (Trauma Informed Practice).
* Experience of achieving positive outcomes with clients using Motivational interviewing and assertive engagement.
* Working alongside Volunteers to manage risk and deal with any support issues.
* It is essential you have access to a car and be able to drive due to the nature of the role.
