An exciting opportunity has arisen for a Band 7 Adult Clinical Nurse Specialist within the field of Haemoglobinopathies. The post holder will work as part of a comprehensive multidisciplinary team within Sickle Cell and Thalassaemia Services at Homerton NHS. The service is integrated, and the post holder will be required to work across the community and the acute setting in rotation with the nursing team to meet service needs and patient demands.
The Service is looking for a highly motivated, dynamic, enthusiastic, and experienced nurse to work in conjunction with a multifaceted multi-disciplinary team focused on providing high-quality patient care. The post would suit someone with excellent communication skills and a good understanding of change management. The post holder will be required to work autonomously as well as part of the team, role modeling and acting as an expert in their field. There is potential to develop this role for the right candidate.
The successful candidate will provide specialist nursing care, education, and support for individuals with Sickle Cell & Thalassaemia.
The Homerton serves a significant Sickle Cell and Thalassaemia patient population and treats patients referred from inside and outside the City and Hackney area. Therefore, it is essential that the post holder has a good knowledge and understanding of the implications and nature of the disease and its processes.
The post holder will work with the Lead Nurse and members of the Sickle Cell & Thalassaemia team, including the wider multi-disciplinary team, to provide comprehensive care for people with Sickle Cell & Thalassaemia Disorders within the London Borough of City and Hackney. It will be essential to work across the community and the hospital setting to improve standards and equity of care while promoting an integrated care pathway, suitable discharge planning, and improving patient outcomes.
The post holder will be expected to comply with National and Local standards and guidelines in the management of Adults and Children with Sickle Cell & Thalassaemia disorders. Active health promotion and outreach work will promote the work of the sickle cell team and raise awareness in the local community.
Effective communication within the team is essential, as well as effective liaison with other agencies such as social care, community nursing teams, and safeguarding.
